If enabled Reality Mode in the Mod Config Menu, this item will spawn in the starting room (only in Hard mode). On use No effect if not used in the very beginning of the run.

Triggers R Key.

+1 broken heart.

Enemy won't spawn any reward on death.

Hearts have 33% chance to become

Rotten Hearts.

×1.2 enemy HP.

Bosses grants armors equal to stage level.

Deals becomes more expensive every floor.

Some characters will have special effects:

• Isaac or Tainted Isaac: Restart as Isaac. Spawns The D6 and starting with Birthright. If Isaac has more than 10 items, future item pedestrals will be replaced by random trinkets. Upon taking damage, there is a chance to drop a random item on the ground (100% chance if Isaac has more than 10 items). The D6 can reroll trinkets.

• ??? or Tainted ???: Starting at Chest level, and spawns

Golden Key. Clearing a room guarantees a Locked Chest. ??? becomes stronger in larger rooms. Defeat ??? boss to unlock the achievement.

Azazel or Tainted Azazel: Restart as

Azazel. All special rooms has a 1/3 chance to become Devil rooms ( Number Magnet layout), 2/3 to disappear.

• Eden or Tainted Eden: Most difficulty effects are doubled.

Lilith or

Tainted Lilith: Restart as

Tainted Lilith. -0.1 speed. Every 10 room cleared, spawns a random familiar item. 20% chance to grant 1 broken heart upon room clear. Upon entering next floor, remove all familiar Lilith has, and spawns random Angel Room items.
